The assessment process establishes your current level of skills and the causes of the difficulties you are experiencing. The assessment also takes into consideration your personal circumstances such as your environment, your cultural needs and your support network. Then based on your values and the areas you identify that you would like to address a plan can be agreed. This may entail treatment to enable you to regain skills or addressing environmental barriers which prevent you from carrying out activities.
